A little preparation can make your visit easier and help your clinician understand your concern. You do not need to diagnose yourself before calling.

Your visit checklist

  • Health card: bring your valid Ontario health card if you have one. If you do not, ask reception about eligibility and fees before your visit.
  • Medication list: include prescription medicines, over-the-counter products and supplements, with doses if known. Mention allergies and reactions.
  • Relevant information: bring recent test results, reports or paperwork related to the concern, if available.
  • Symptoms: note when they started, whether they are changing and what you have tried.
  • Questions: write down what you want to ask, including how to obtain results or arrange follow-up.

Before leaving home

Call Eliva Care at 289-409-0090 to confirm walk-in availability. Tell reception if your visit involves a child, a form or a procedure, since a separate appointment or fee may apply.

Find the address and other details on our Milton walk-in clinic page. Wait times and capacity can change during the day.
